1. Obtain your FREE Patient Information Kit by calling us toll free at 1-800-728-0522 or by downloading the Patient Forms from our web site.

Our FREE Patient Information Kit contains all the required All Canadian Meds Patient Forms you are required to complete in order to ensure timely processing of your order. These forms can be downloaded directly from our web site by clicking on the icons to the left or by calling our offices toll free at 1.800.728.0522 and requesting our FREE Patient Information Kit be delivered directly to your door.

2. Complete the required All Canadian Meds Patient Forms.

Once you have obtained our All Canadian Meds Patient Forms, use the enclosed easy-to-follow checklist to ensure you have properly provided us with all the required information. The forms should be signed and dated and be sure to provide us with accurate credit card information. Failure to do so will delay processing of your order.

3. Mail or fax the completed forms along with the original prescription(s) from your doctor to our offices.

Once you have completed the tasks listed on our easy-to-follow checklist, you are ready to return your All Canadian Meds Patient Forms and order request to our offices. It is very important that the original prescription(s) from your doctor be included in this order, otherwise we will not be able to fill your request! Furthermore, both the All Canadian Meds Patient Forms and the original prescription(s) should be returned to our offices at one time. Sending these items in separate envelopes will again delay the processing of your order and will hold-up delivery to your residence. You can return both items together by mail or fax:

Once we receive your All Canadian Meds Patient Forms and the corresponding original prescription(s) from your doctor, we take care of the rest!

First, your forms are carefully reviewed by our team of pharmacists and physicians to ensure you receive the highest level of care. Next, payment is processed and the order is filled and sent to our shipping department for proper packaging and delivery. Before being sent out, you will receive confirmation of your order from All Canadian Meds by either phone or e-mail. Upon confirmation, please allow up to 21 days for the delivery of your medications. All medication prices are listed in US funds and a shipping charge of $15US will be applied to each package. At this time, our dispensaries will only accept Visa and MasterCard. Personal checks and money orders are NOT accepted since the time required to guarantee funds significantly interferes with our ability to provide you with the highest level of service.
